What to bring with you to your appointment

  • Your current insurance card(s). We will need to make a copy of your insurance card at every appointment in order to assure that your insurance will be billed correctly.
  • A snack or lunch to eat, especially if you are diabetic.
  • A driver to take you home, as your eyes will be dilated during your appointment.
  • A completed copy of the following new patient forms:

Retina Northwest takes seriously our responsibility to protect your personal health information, and to coordinate your care with referring physicians, family and health insurance plans in accordance with applicable laws. 

Under a federal law called the Health Insurance Portability and Accountability Act (HIPAA), covered health care organizations across the nation, including Retina Northwest, must have and adhere to a written Notice of Privacy Practices, provide you with a copy of this Notice, and receive a signed acknowledgement from you that you have received a copy of the Notice.

A special note regarding insurance: Referrals

Many health insurance plans require referrals before their members obtain specialty medical services like the care provided at Retina Northwest. Our referrals staff works diligently on your behalf to obtain these when needed; however, they cannot do so if they are lacking your insurance information prior to your appointment.

Please help our staff to help you by ensuring that your complete and most current insurance information is sent over to us by your referring physician's office, when they make your appointment. If you need to provide us with this information directly, please call our scheduling department at 503-274-2121 as far in advance of your appointment as is possible.

What to expect, and how long your appointment will last

It is very likely that the doctor will need to perform diagnostic tests and obtain results before completing your visit. It is also possible that after reviewing your test results, your doctor may recommend that you receive treatment on the same day as your visit. Please allow 2 to 4 hours for your appointment.
